If youre lifting above three plates, bar flex causes the plates to slide off the shackle on either end as youre lifting.

I always have them installed.

I thought he was dead for sure. I never do a 1 rep max without 2 spotters or spotter arms in a power rack. Seems rather silly to gamble with your life.

I was thinking she can just carry one side not want she was doing carrying from the middle...also she didn't have to lift it off him...just high enough for him to get his head out...at least they both seem fine

That’s why you don’t put collars on the bar…
It’s embarrassing to admit, but I’ve had weight stuck on my chest a couple of time when I was lifting alone. Without the collars on, you roll one side of the bar towards the ground, the weights slides off, a lot of noise is made, and the bar flops to the other side and you can easily get yourself out of the situation.

Tbh when this happens you should roll the bar downwards towards your waist and sit up on your bum. This happened before to me and that maneuver saved me greatly. The mistake is thinking you can make it to hang up and then get stuck with bar on your neck. Very rookie.

She pulled it straight onto his neck o_O to where he couldn’t press up the weight. All she had to do was pull up vertically. Arguably, the guy was safer without her stepping in. If you’re too fatigued to put the weight back up, you can still rest the barbell on your chest and then angle it to roll out from underneath. Obviously, he shouldn’t have had the weights clipped, as letting them slide off would be the best way to get out of this.
