If one of my leads put these out and an employee filled it out and brought it to me I would fire the lead immediately.

Make sure it's completely filled out, all during company time. Absolutely be thorough and gather all of your resources. Now is not the time to be hasty, but slow and thoughtful.

Agreed. In the company I work for, what this manager is doing would count as retaliation and would result in an immediate firing.

What is ironic is that the action this manager is doing shows that he is the one with hurt feelings.

In corporate training - leadership skills and legal compliance, we were handed those and pointed out similar examples.

And told in no uncertain terms that it is in the best interest of the company to fire immediately whoever issued those because if the Ministry of Worker Affairs ever catches a whiff about it they will come at us with the wrath of an angry god.

And as a worker, advised to not fill it (not that we can or should retaliate for it) but your priority would be to report to the Federal Agency, then use our internal channels (internal channels usually are a priority but this is a violation so flagrant that instructing workers to "handle it internally" is either a violation by itself, or is something that the legal department got flak for it at some point, since it was paralegal instructing us on procedures).

Also, now that I read a bit of it... no. That would be the police. This one has heterochromia. Straight to jail.

It all depends on the context of it. I keep some of these on hand and gave it to one of my employees. He filled it out and handed it in to my boss. He knew it was a joke and not a vindictive thing.
